The Case Manager assists with social services functions including communication with referral sources, families, physicians, and schools, ensuring patients/families are scheduled after care, short-term and long-term family therapy, providing information and referrals as needed and conducting wellness checks.
QUALIFICATIONS
Education: Bachelors Degree from an accredited college or university, in Social Work or a clinical related mental health field.
Experience: Two (2) years' experience in a mental health care facility preferred; must have strong communication skills.
Additional Requirements: CPR certification and HANDLE WITH CARE training within 90 days of employment and prior to assisting in a restraining procedure.
Hours of Work: May be required to work flexible hours.
