The Lead Social Worker is a member of the Care Management team who collaborates directly with the care manager and interfaces with the patient, and/or family and/or the patients representative to assess the patients post-hospital care needs and ensure the patients transition to the least restrictive care environment; This includes assessments for and referrals to acute rehabilitation, extended care facilities, durable medical equipment providers, home health agencies and other resources as necessary. Additionally as an independent professional, the Social Worker assesses the patients psychosocial and continuum of care needs; Interventions may include, but are not limited to, crisis intervention, consultations with staff physicians and psychiatrists when indicated, Adult or Child Protective Services referrals and custody or guardianship issues; This screening may require patient assessments for substance abuse, domestic violence and bereavement; Interventions may include family support, crisis intervention, access to pharmaceutical indigent patient programs and resource management. The Social Worker adheres to all Norton Healthcare policies and applies his/her educational and professional expertise in the delivery of quality care. The Lead Social Worker will support the social work/care management team by mentoring, educating, and serving as a liaison between social work and other disciplines.
